Managers can choose Forms, then start from a template or a blank form. Changes save as you type, and What people see shows the form as you build it.
Question types
- Short answer and Long answer for writing.
- Multiple choice and Dropdown to pick one option.
- Checkboxes to pick several.
- Rating, on a scale to 5, 7 or 10, with your own labels.
- Date, Email and Number.
Mark any question as required, and duplicate or reorder questions as you go.
More options for a question
- Add help text under the question.
- Add an Other answer that people can write in.
- Shuffle the options for each person.
- Show a question only when an earlier question has a certain answer. Hidden questions aren't required, and their answers aren't saved.
Text blocks and pages
Add a text block for a heading or instructions, and a page break to split a long form. People then see a progress bar, with Back and Next.
Make it your own
Under Look and feel, choose a colour, a cover picture or your own image, and the send button's text. In Settings, write your own thank-you heading and message, and add a button linking to another page.
Who can respond
- Anyone with the link: no sign-in, and no name is recorded unless they write it in an answer.
- People signed in with Discord: their Discord name is saved with their response. You can limit this to one response per person.
Sharing
Every form has its own random link, like /forms/h7Rt2mKqW9xLc4Pz. Make a new link stops the old one working. To close a form, turn off Accepting responses, set a closing date, or set a response limit.
After a course
In the course builder, choose a Feedback form after passing, and people see a link to it when they pass.
Don't use forms to ask for passwords, or for payment or other sensitive details.
